Job Summary: Integrate collaborative care planning for individual patients and implement nursing standards in the processes of care

• 3+ years CURRENT MS - [Required]
• EPIC EMR experience - [HIGHLY Preferred]
• CHARGE EXPERIENCE [REQUIRED]
• Previous Travel - [Preferred]
• Perm address of traveler must be included in profile
• Traveler must have their own vehicle for this need
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer. Please obtain this information prior to submittal. Requests after offer may lead to rescinded offer
• TRAVEL PAIRS WILL BE CONSIDERED FOR THIS NEED ONLY IF THEY CAN WORK SEPARATE SHIFTS

Profile Requirements:

• Work History
• Skills Checklist (pertinent to position), current to 1 year
• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications - Must be uploaded with profile

CERTIFICATION REQUIREMENTS :

• ACLS (AHA)
• BLS (AHA)

STATE LICENSE REQUIREMENTS :

• New Hampshire

ADDITIONAL LICENSE REQUIREMENTS : License and Certs must be active and in good standing.
Weekend Requirements : Every other weekend/Holidays required if needed
On Call Requirements : None
